Non arteritic anterior ischemic optic neuropathy

It occurs commonly in patients between 45 and 60 who are hypertensive, smokers or have high serum lipids.

Symptoms:

Moderate to severe visual loss.

A) Color vision:

It's impaired in proportion to the visual acuity in contrast to optic neuritis in which Color vision is markedly impaired out of proportion to the visual acuity.

B) Fundus: examination shows edema of the optic disc that may be diffuse or affecting one sector.

Also the disc becomes surrounded by splinter shaped hemorrhages.

Investigations:

Field vision: field defects in the form of altitudinal scotoma, arcuate scotoma and occasionally centrally if papillomacular bundle is affected.

Blood picture and serum lipids.

Fate:

Gradual resolution followed by optic nerve pallor occurs in most patients.
